한국 노인의 식생활 지침 실천도왿� 알츠하이머형 치매의 관련성 (Association between compliance with dietary guidelines and Alzheimer's disease in Korean elderly)

우리나라의 노인 대상으로 식생활지침 실천도 점수에따른 치매 유병률의 차이를 분석한 결과, 치매 노인군이 정상노인군에 비해 나이가 유의하게 많았고, 성, 체격, 그리고, 식사속도, 식사를 거르는 횟수, 편식 등 식생활 특성에는 정상 노인군과 유의한 차이가 없었다.
식생활지침 세부 실천지침 중에서 다양한 식품군 섭취, 규칙적이고 안전한 식사, 음주, 가벼운 운동부분에서 치매노인군의 실천도가 정상노인군에 비해 낮았고, 그 외의 문항에서는 유의한 차이가 없었다. 식생활지침 실천도 점수가 높을수록 치매 유병률이 낮은 경향을 보였다.
요약하면 어르신을 위한 식생활지침의 실천도가 높을수록 알츠하이머형 치매의 위험이 감소하므로, “어르신을 위한 식생활지침 (보건복지부, 2011)” 실천도를 높이는 것이알츠하이머형 치매 예방에 도움이 될 수 있으며, 향후 알츠하이머형 치매 예방을 위한 식생활 지침의 기초자료로 활용할 수 있을 것으로 사료된다.

Purpose: Confronting the growing burden of dementia requires understanding the causes and predictors of dementia in order to develop preventive strategies. In Korea a large proportion (71%) of dementia is Alzheimer's disease, and the risk factors have not yet been elucidated. Dietary factors may be possible risk factors, however research on the relationship between Alzheimer's disease and dietary behaviors has been insufficient. The purpose of this study was to investigate the association between compliance with the “Dietary Guidelines for Elderly (The Ministry of Health and Welfare, 2011)” and Alzheimer's disease among Korean elderly. Methods: Elderly persons who visited a University hospital or a dementia center of Seoul and agreed to participate in the examinations were selected. Among 277 subjects, 89 Alzheimer patients were selected with diagnosis and 118 subjects were assigned to the control group. Diagnosis of Alzheimer's disease was based on Alzheimer's disease criteria of Diagnostic and Statistical manual of Mental Disorders, 4th edition (DSM-IV) and criteria of National Institute of Neurological and Communicative Disorders and Stroke-Alzheimer's Disease and related Disorders Association (NINCDS-ADRDA). Information on the general characteristics, health related behaviors and compliance with the dietary guidelines for Korean elderly was collected by face to face interview using a structured questionnaire.
Anthropometric variables were measured during the survey. Results: Total compliance score of dietary guidelines was significantly lower in the Alzheimer's disease group than in the control group (p = 0.0001). The odds ratio of Alzheimer's disease was significantly decreased in the group with the highest dietary guideline compliance score (OR = 0.47, 95 % CI = 0.18~1.09) compared to the group with the lowest compliance score. Conclusion: The results indicate that increasing compliance with the dietary guidelines could be an effective strategy to decrease the risk of Alzheimer's disease among Korean elderly.
